White Oak Psychiatric Services has helped thousands of people with a wide range of mental health needs. In order to best serve you, we offer the following information:

New Patients

Download our new patient form that you can print, fill out and bring with you. Also, please remember to bring a list of your current medications, both prescription and over the counter.

If you will want records sent from a previous doctor, download and complete this HIPAA release form. You may fax it to our office or you can bring it. For children, use a SNAP form for parents and teachers to complete.

Be sure to bring identification such as a driver’s license and insurance information.

Appointments

Please arrive for your appointment on time. We know life can be hard and, especially today, there is a lot going on. But our doctors help many patients throughout the day, and we have to respect our schedule in order to serve everyone.

Medication Refills

If you run out of medication, please call your pharmacy to check for refills before calling us. As a rule, we cannot respond to pharmacy requests for refills.

Urgent Situations and Emergencies

If you are facing an emergency, call 911 for emergency services or go to the nearest emergency room. We suggest that you keep the National Suicide Prevention Lifeline number (1-800-273-8255) in a location where you can easily find it.
